Dear parents,
Several prospective parents have asked if children at our school can have access to before and after school care. I would like your help in gathering as much information as possible about present arrangements, and to find out your feelings about provision in the future.
Please fill in the following questionnaire and return it to school as soon as possible:-

1. How does your child get to school?
I usually bring my child myself ?
I take turns with a parent of another child ?
I employ a carer to bring my child ?
2. How does your child get home from school?
I usually collect my child myself ?
I take turns with a parent of another child ?
I employ a carer to collect my child ?
3. Before and after school care for my child is

4. If there was provision at school for children to be looked after before school;

Our open morning for parents of children starting school next September takes place next Wednesday. I think it would be a great pity if parents are unable to send their children to this school because of child care problems. I should be very grateful if you could return this questionnaire by Monday, so that I can tell new parents your ideas.
